CCA questions

I have received a lot of information regarding the CCA. If anyone can answer any of the following questions, I would greatly appreciate it. I've taken the exam and failed and the "trivial" questions are what got me. which is what some of these are: 1. 1st, 2nd, and 3rd degree burns of the chest wall. What is coded? The highest degree of burn is coded, correct?
2. When a patient is admitted who coordinates the care? (a) Intern, (b) resident, (c) primary care or (d) attending. Is Attending correct?
3. How many ICD-9-CM codes can be reported on a CMS-1500 claim? 4? I’m reading 4 in some places and 6 in other places. Although I know there are only 4 places on the claim form. 4. How do you code a bilateral procedure on the lower extremities when the book does not specify unilateral or bilateral? 5. Coder’s child seen for injury. Coder accesses x-ray report to see if there was a fracture. This access was discovered on audit. What should the coding manager do? Options: (a) Nothing, (b) Immediately fire the coder, (c) Report the access to privacy officer, (d) Get the coder to sign authorization. 6. Doctor requests wife’s record. What should be done? (a) Give the doctor the record (b) Inform the doctor you need signed authorization for the wife. 7. Coder finds incident report in the chart. What should they do? 8. Diagnosis of sinusitis. Documents has vitals, neck and ______________. What is missing? HEENT? 9. What would be recorded on a graph? (a) Medications (b) History (c) Vitals
10. Employee given new manual of policies and procedure to sign. How often would this happen? 11. These are all methods of paying a Medicare physician for services EXCEPT: (a) APC (b) RBRVS (c) fee-for-service
12. Patient seen for _____________ of sigmoid. What is the appropriate test? Is the answer Flexible colonoscopy?

These are some good questions. 1)You code the highest degree is correct. 2-4 not 100% sure so I won't say anything about those. Will look them up. 5)I believe "C" although "B" is a huge possibility. 6)Is the doctor providing care to his wife? Not 100% but I would say "B" authorization needed 7 & 8 not sure 9) Vitals 10)What are your choices? Seems like it would happen upon employment and thats it. 11) Not sure 12)Yes, flexible colonoscopy 13 & 14 I have to look up and get back to you.

thank you for your response. #10 was given to me with no choices. And again, I know these seem very trivial but some of these WERE on the exam with some very tricky answers to accompany and I can't locate the specific answers to these questions online, in my textbooks, or anywhere. Be able to identify PCP in a scenario 17. What a consultation is a what a consult is? 18. PHI – highest level of security? 19. Types of authorization to give out PHI? 20. Whether you can legally fax a medical record or not? 21. DRG grouper 22. What increases DRG assignment? 23. Quantitative vs. Qualitative? I understand the difference, but not sure how this would be used in an exam question. 24. What are Emergency Department codes based on? (history, physical exam, medical decision making, time)??? 25. Query form. How to tell if it’s not right? If not right, what is missing? 26. APGAR is found in what report? 27. Can you fax medical records to a doctor from another city? 28. Is it okay to query the physician regarding adding a diagnosis based on documentation and state that adding the diagnosis will increase reimbursement? 29. Creatinine and Toponin labs are ordered for what diagnosis? 30. The person with the highest access to patient information? Nurse manager? 31. ? Volts of electrons were directed at three times at 2 treatment areas and 1 port. What kind of report would you find this on?

Can anybody help me with these CCA questions?
