CapitalCare Medical Group is a physician-owned medical practice with over 500 employees including more than 150 healthcare professionals. The group offers primary care services in Family Practice, Pediatrics and Internal Medicine throughout the Capital District. CapitalCare also provides services in Endocrinology, Pulmonary and Sleep Medicine, Developmental-Behavioral Pediatrics, Allergy, Asthma & Immunology, Nephrology, medical nutrition therapy, and comprehensive diabetes education and operates a state-of-the-art clinical laboratory. CapitalCare welcomes new patients and participates with most major health insurance carriers.

We have a new opportunity for a full-time RNCase Manager to join our growing team at our Central Business Office in Colonie!

Responsibilities include but are not limited to:
Work within the scope of practice for a Registered Professional Nurse as defined by the New York State Education Department.
Identify patients who can benefit most from case management services.
Complete a health and psychosocial assessment, recognizing the cultural and linguistic needs of each patient.
Identify problems or situations that would benefit from case management intervention.
Identify patients' immediate, short-term, long-term, and ongoing needs, and develop appropriate case management strategies to address those needs.
Demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess a patient's status through collection and interpretation of relevant data.
Maintain a working knowledge of community-based services and resources, and the process for referring patients for services.
Seek input from other care team members regarding the patient's plan of care.
Develop and implement an individual Plan of Care for each patient in his/her caseload and evaluate and revise the plan as needed.
Maintain contemporaneous patient records such that problems, plans, actions and goals are accurately and clearly stated, and changes are reflected as they occur.
Initiate (or refer as appropriate for) patient education based on identified learning needs of the patient and/or those providing care.
Maximize the patient's health, wellness, safety, adaptation, and self-care through effective case management, patient satisfaction, and cost-efficiency.
When appropriate, terminate case management services based upon established case closure guidelines.
Facilitate coordination, communication, and collaboration with the patient and other stakeholders in order to achieve goals and maximize positive patient outcomes.

Adhere to applicable local, state, and federal laws, and CapitalCare policies governing the patient, patient privacy and confidentiality, and act in a manner consistent with the patient's best interest.
Obtain informed patient consent before case management services are implemented.
Behave and practice ethically, adhering to the tenets of the code of ethics that underlies his/her professional credential.
Advocate for the patient with respect to service delivery, benefits administration, and policy making.
Be aware of, and responsive to, the cultural and demographic diversity of the population and of specific individuals.
Integrate factors related to quality, safety, access, and cost effectiveness in assessing, monitoring, and evaluating resources for the patient's care.
Maintain familiarity with current research findings and apply them, as appropriate, in his/her practice.
Perform other duties as the business dictates.

Knowledge/Skills/Abilities:

Work with all departments and sites in a collaborative manner.
Possess excellent communication skills.
Have of TQM concepts, methods and tools.
Have of medical records and clinical care processes.
Demonstrate ability to work effectively as a member of an interdisciplinary team.
Possess excellent organizational skills and strong attention to detail.
Have proficiency in the use of the Microsoft Office suite, especially Excel, Outlook and Word.
Have familiarity with medical records and clinical care processes.

Qualifications/Requirements: Possess a current license to practice as a Registered Professional Nurse in the State of New York. Two years' work experience as a registered professional nurse, preferably in a primary care setting. Case manager experience preferred. Must be able to travel to office locations. Must have a clean, valid driver's license and current auto insurance and registration.
