Responsible to facilitate mental health rehabilitation interventions and supports necessary to assist individuals with mental illnesses to achieve rehabilitative, resilience and recovery goals primarily in a person’s own environment. This worker will provide direct service to our members through individual contact mainly in the community, frequently in the member’s home. This worker will support our members in their recovery through individual support, skill building, and case management. This worker will advocate with collateral sources to assist members to receive the services and support they need to work on their recovery and independence goals. This worker will always respect members’ right to determine the course of their lives. This worker will communicate well as part of an interdisciplinary team.
Must have a valid Driver's license and good driving record.
Must have 100,000/300,000/100,000 auto insurance.
Bachelor's degree, or a minimum of five years supervised experience in mental health or human services or the ability to meet Office of Mental Health standards as a Mental Health Professional required.
This worker will be computer literate, well organized, and able to fulfill all agency documentation and revenue goals.
