Responsible for implementing individual and group activities based on the needs and interest of the resident(s).
Key Responsibilities/Essential Functions:
Escorts residents to scheduled day, evening and weekend activities within the ministry.
Assists residents during activities.
Attends and assists residents on ministry outings.
Implements plan of care in relation to activities as appropriate to the level of living.
Documents and manages “Activity Daily Sheets” for activity participation.
Participates in the organizing and scheduling of community events.
May perform other duties as assigned or requested.
This position has no management responsibilities.
Education and/or Experience
High School Diploma or equivalent is preferred
1-2 years experience working with the elderly
Minimum of two years unencumbered driving record required, preferably with an older adult, physically challenged population.
Possess a personal presence characterized by a sense of honesty, integrity and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals and values of Via Christi Health.

Exhibits well developed verbal and written communication skills, and is able to professionally receive and follow oral instructions in English.

Demonstrates compliance with all Via Christi policies and procedures.
Basic computer knowledge required.
Working knowledge of Microsoft Word and Excel required.
Certificates, Licenses, Registrations
Must have a valid driver’s license in the state which employed.
Valid Certified Nursing Assistant (CNA) certification in the state which employed.
CPR Certification preferred.
