The Coordinated Care/Quality Assistant role is to provide support to the organization by data management, receiving and processing information and enhancing communication.



  • Assist collect, measure and document metrics for the Delivery System Reform Incentive Payment (DSRIP) transformation wavier.
  • Prepare and manage correspondence, reports and documents.
  • Organize and coordinate meetings.
  • Record, type and distribute meetings minutes.
  • Maintain schedules and calendars.
  • Set up and maintain filing systems.
  • Communicate verbally and in writing to answer inquiries and provide information.
  • Serve as a liaison with internal and external contacts.
  • Coordinate the flow of information both internally and externally.
  • Operate office equipment.
  • Manage office supplies.

  • Qualifications:


  • Required: High School Diploma


  • Relevant training or work history
  • Experience with Microsoft Word and Excel required.

  • Effective written and verbal communication and negotiating skills with all levels of internal and external customers.
  • Must demonstrate positive relationship building with co-workers, medical staff, patients, and other members of the clinical care team.
  • Must demonstrate the ability to effectively implement changes
