The anticoagulation management service AMS Coordinator responsibilities are to provide one on one assessment and teaching to patients on Coumadin/Warfarin therapy based on established protocols under the supervision of the Chief of Cardiology or his/her designee. Function as a resource to other health professionals in order to ensure continuity of care. Collaborate with other personnel within the department of Cardiology as needed. Minimum two years experience in ambulatory setting with telephone patient management skills. Must have previous experience providing care to patients requiring anticoagulation therapy using Coumadin/Warfarin and/or patients with cardiac disease. Demonstrate understanding of complex medications, food and other interactions with anticoagulation typically given to adult patients. Knowledge of evidenced-based medicine in cardiovascular nursing required. Previous experience using clinical protocols to make decisions for adult patient required. Demonstrate excellent verbal, written and computer skills required. Portuguese or Spanish speaking preferred. Graduate of an approved school of nursing with current licensure to practice in the state o Massachusetts. BLS certification required. ACLS certification preferred.
