Under the program model, care managers, or 'Nurse Consultants', lead an interdisciplinary team that also includes social workers, rehabilitation therapists, nurse practitioners and nutritionists. These clinical specialists collaborate closely with program member/patients, caregivers, community physicians, and other providers to achieve a common goal: helping member/patients remain independent at home and in their communities for as long as is safely possible.We are seeking talent for the opportunity described below.

Responsible for overseeing, coordinating, planning and organizing the day-to-day operations and administrative matters to support the goals and objectives of the Department/Program and Agency. Assists in the coordination and management of administrative matters and handles confidential information. Works under general direction.

Education: High School Diploma or the equivalent required. Bachelor's Degree in Business Administration, Health Care Administration or other related field, or the equivalent preferred.Experience: Minimum of three years progressive administrative, business or office experience required. Advanced personal computer skills, including Microsoft Word, Excel required. Effective oral, written communication and interpersonal skills required. Ability to multi task in a fast-paced environment required.
