Less than 14.5% of VNA patients are readmitted to the hospital, which puts us in the top 7% nationally. In addition, less than 6% of VNA patients are readmitted to the hospital within 30 days of discharge from multiple facilities, compared to a nationwide average of 11.7%. Admissions Coordinator is a clerical position responsible for performing intake of patient referrals by obtaining sufficient information to make first visit, processing referral information and distributing it to scheduling and other appropriate departments to provide prompt customer service. Processes referrals using the Agency's established procedures and meeting quality and quantity performance standards. Enters data accurately into computer terminal as information is received via telephone, from physicians, hospital coordinators, clients, families and other sources. Asks callers clarifying questions to identify needs, verify demographic information, diagnosis, details on supplies, etc. Provides overview of agency services and recommends those consistent with clients' needs. Provides complete, accurate referral information to schedulers and other departments to ensure effective communication and efficient service delivery. Verifies insurance coverage, if applicable. Provides backup to Insurance Verification Coordinator as needed. Provides basic information on Durable Medical Equipment and makes referrals. Applies knowledge of Agency resources to provide high quality customer service, by providing general overview of Agency services as specific program details to callers, participating in continuous quality efforts as appropriate, and recommending services which are consistent with patients' needs.
• High school graduation or equivalent required. Minimum two years as a medical transcriptionist, admissions/intake coordinator, or other work experience in the medical field. Data entry skills to accurately input alpha numeric data into computer terminal.
• Sufficient knowledge of medical terminology to understand and interpret physician's orders, written or spoken. Able to read and interpret physicians' orders, notes from caregivers, and entries on computer screen as well as respond to them, verbally and in writing.
• Able to calculate drug dosages, mix insulin, measure intake/output when conducting home visits.
