JOB TITLE: Assistant Director, Medicaid Service Coordination
Reports to: CEO
Hours/Days of Work : Full time, 37.5 hours per week, Monday - Friday. Additional evening and weekend hours as required by caseload.
Education/Experience: Four-year college degree required preferably in Human Services and 3 years experience working with individuals with developmental disabilities. 3 years experience as a supervisor of MSC required.
MAJOR RESPONSIBLITIES - Assistant Director, MSC
Provide oversight of delivery of case management services through Medicaid Service Coordination, home and community based services waiver programs.
Ensure proper documentation for Individual Case Files as required by Federal and State regulations governing Medicaid Service Coordination and Home & Community Based Services waiver programs.
Ensure all personnel policies and procedures are adhered to.
Make referrals to appropriate service providers based on Individual Service Plans, consumer requests, guardian/advocate requests and/or need.
Train and coach staff on the delivery of the Planning Process to ensure Valued Outcome's are achieved and personal goal's and dream's are met.
Monitor the intake/referral list and assign MSC's as appropriate.
Monitor program budget and ensure it is within target.
Fulfill all State and agency training requirements, and ensure proper training of subordinates.
Act as liaison between agencies, agencies and consumers, and agency departments and ensure good communication.
Monitor and respond to consumer satisfaction.
Communicate agency initiatives to MSC Department and develop plan for attainment of initiatives.
Participate in various agency committees.
Act as liason between person receiving services and the court to facilitate the Guardianship process.
Facilitate Guardianship Board Committee.
Responsible for communication and implementation of policies and processes involving Guardianship.
Serve as agency resource in times of end of life decision making.
