The Lifestyle Wellness Coach provides patient education, peer support, and advocacy for persons with conditions impacting wellness. This position facilitates designated Community Health Center (CHC) patients' and residents' understanding of health issues and available resources as well as their ability to take control over their own health needs. The Lifestyle Wellness Coach may also provide/fill other patient advocacy roles within the clinic as well as assist in program management of other health education initiatives.
-High school diploma or GED required with a current credentialed fitness certification or one obtained within 6 months of employment, OR Bachelors degree or higher in a fitness related position
-Minimum of 2 years experience working in social services, health/wellness, coaching/mentoring or related area
-Familiarity with healthcare environment preferred
-Familiarity with health/wellness terms, objectives, knowledge
-Must be fluent in English and Spanish (ability to read, write, and speak both)
