CADC Clinician

CTT is mandated to provide service to those individuals within the mental health system who present the greatest degree of severity of symptoms as evidenced by their multiple physical, psychiatric, and legal conditions, extensive use of services, and lack of follow through with treatment. This population requires the highest level of medical necessity as defined by the state Adult Environmental Matrix. The Teams that provide multidisciplinary clinical review and assessment of individuals who may be young adults, heavy users of service, or actively involved in the criminal justice system and may also have co-occurring behavioral health diagnoses can be generic or specialized. The CADC provides supports to all individuals assigned to his/her team as a part of the Community Support Program of Philadelphia. The CADC will assist the individual in developing skills required to achieve sobriety, enhance his/her comfort and abilities to function as a productive member of the community and will ensure that the mission, goals and philosophy of the CTT are operationalized within the team. The incumbent works with individuals in their community environment assisting them in understanding, acquiring and maintaining independent living skills in the areas of: (a) their daily living situation; (b) interpersonal skills and social support/network building; (c) leisure and recreational support/skill development; (d) maintenance and enhancement of physical and mental needs, with particular focus on substance abuse issues; (e) obtainment of benefit entitlements and the skills to manage same; (f) vocational and educational needs; (f) accountability in fulfilling their legal obligations in order to remain in the community.

Duties and Responsibilities:

Provides comprehensive assessments for assigned individuals with particular reference to substance abuse issues, including writing comprehensive reports.

Participates in daily multidisciplinary team meetings to review progress on cases in order to ensure communication and comprehensive approach to treatment, including collaborating with external teams to ensure coordination between systems; participates in CADC staff group supervision as required for skills enhancement.

Provide input in case reviews to court personnel as deemed appropriate and necessary.

Provides individual and group treatment to address addictive behaviors and issues, utilizing a psycho-educational approach.

Views individuals from a strengths approach in preparation of treatment plans, with a focus on recovery.

Monitors service delivery to individuals to ensure appropriate level of service given need as defined by Environmental Matrix score.

Establishes networks and liaisons with community service providers to resolve problems and overcome barriers to service, including attending linkage and treatment team meetings as needed; provides advocacy mechanism to ensure that individual needs are met.

Monitors individual’s treatment and progress during hospitalizations by participation in in-patient treatment team and discharge planning meetings.

Maintains and updates community resource files, particularly around substance abuse resources but also behavioral, forensic and young adult population treatment resources, and provides information and shares with team and CTT community.

Completes all required reports and other paperwork in timely manner.

Participates in supervision format for CADC/MISA staff with Assoc. CM Director.

Participates in Clinical Care, Open Forum and other required agency meetings; attends in service training in order to meet mandated training hours.

Plans and assists individuals to increase community tenure, enhance quality of life, and attain highest level of independent functioning.

Provides after hours work including: a) participation in weekly on call rotation schedule to permit 24 hour/7 day a week access to service; b) and participation in evening and weekend shift work schedules required for delivery of services to CTT’s caseload.

Performs other duties as required.

Skills Required:
Must have excellent verbal/written communication skills and work well with people; good, creative problem solving skills; excellent clinical skills including assessment, individual and group therapy; knowledge of behavioral health and criminal justice systems, community resources; ability to work independently, and be flexible/adaptive in handling changing priorities in a fast paced work environment; computer skills preferred.

Education and Experience:
Bachelor’s Degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling, education with CADC, CAADC or equivalent and 3 years experience in clinical and therapeutic work. Expertise preferred in behavioral health, PCP training and expertise in one or more of the following: criminal justice system, HIV/AIDS or geriatric issues.

Essential Functions:
Ability to sit for at least one hour at a time. Able to climb stairs when visiting consumers in their residence. Able to lift up to 20 pounds when assisting consumers with residential moves.

Other Job Requirements:
Must have a valid driver’s license, auto insurance and access to a vehicle; able to share office space and work as a part of a team; able to work evenings and weekends in meeting responsibilities.
