Provides expert clinical leadership to interdisciplinary care team members in the coordination of clinical care, management of patient outcomes and managing financial responsibilities. The Clinical Care Coordinator has the responsibility, accountability and authority for providing comprehensive care coordination and knowledge to plan, implement, monitor and evaluate the outcomes of care for the designated patient population.
Assess the needs for acute care inpatient services, insuring that care is appropriate and directed to resolve these needs and determine how care can be transitioned to a lower level of acuity as the patients? condition warrants.
Graduate of an accredited school of professional nursing required.
Current license to practice as a RN in the Commonwealth of Virginia required.
Three years recent experience in acute care nursing required.
Case management, UR, Discharge Planning or related experience preferred.
Certification in Case Management and/or applicable Clinical Specialty desired.
*If not certified, certification must be completed within three years.
