Position Summary:

Coordinate all aspects of the cancer patient’s experience from the onset diagnosis, through workup, through treatment, to survivorship.

Job Responsibilities:

Clinical Navigator for Cancer Services:
Nurse Navigators are patient educators and advocates, care coordinators, system navigators, and community ambassadors on a mission to improve the cancer experience for each patient. The Nurse Navigators are a support system for the patient and his/her family at a critical time — after diagnosis and through treatment. A Nurse Navigator is an oncology-certified nurse who is available to guide you and your family through the entire process of diagnosis and treatment at WPHC. The Nurse Navigator is a single point of contact who will be with the patient every step of the way. The nurse navigator assists with scheduling initial tests and consultations. They also collaborate with members of the medical team to ensure information is current and understood by all of the physicians, nurses and other team members. They help the patient and family members understand his or her diagnosis and plan of care.

What can be expected from a Clinical Navigator
The role of the Nurse Navigator is to ease the burden of cancer treatment on both the patient and the family. Nurse Navigators will:
  • Facilitate patient understanding of their diagnosis and the available treatment options.
  • Educate the patient and caregivers of the potential signs or symptoms that may arise over the course of treatment and recovery.
  • Collaborate with all members of the health care team to ensure information is current and available from all of your physicians, nurses and counselors.
  • Connect the patient and family with community resources.
  • Coordinate patient care with all other healthcare provider throughout the system. This includes:
    • Cancer Support Team , which provides: education on disease and treatment; comfort and symptom management; patient and family counseling (including: emotional, social, spiritual, nutritional and financial counseling); referral to community services (home care, palliative care and hospice care); cancer rehabilitation; integrative therapies; and support groups.
    • Cancer Genetics referral documents personal and family medical history and discusses inheritance patterns and risks.
    • Cancer Clinical Trials , which a patient may be eligible for and may recommended as a benefit to you and other cancer patients.
    • Professional Services throughout the system.
Education, Training, Licensure and Experience Requirements:

? BS/BSN required. MS Preferred

? Registered Nurse/ Nurse Practitioner or Physicians Assistant preferred

? Oncology experience / ONS certification preferred

Experience in clinical management of patients with Lung cancer
