In collaboration with Department Supervisor, will help lead, plan develop and implement community health outreach events based on hospital’s community needs assessment findings. Will provide community health education on Diabetes, Heart Disease and other health topics. Will conduct community outreach to identify and schedule people and places that would benefit education classes. Will collect, maintain and report participant data and program outcomes.
Bachelor’s degree preferred
Current BLS card
CPR instructor within 6 months of employment.
LA City Fire Card within 6 months of employment.
Minimum 2 years experience as a Community Health Educator or Health Promoter
Bilingual English/Spanish or English/Korean is required
Strong verbal, writing, communication and presentation skills
Critical thinking and problem solving skills
Strong understanding of Diabetes and Heart Disease
