This is a temporary assignment for the duration of grant funding (over three years).
In collaboration with the team providing clinical care for Aspen Hill Health Center patients enrolled in the Patient Centered Medical Home and other identified Aspen Hill Health Center patients, the peer health coach is responsible for the assistance with the development, implementation, coordination and evaluation of patient tailored wellness care plans and health maintenance activities. The peer health coach will assist with patient navigation that includes addressing the social determinants of health through social services, financial assistance and transportation access; as well as providing educational, social and emotional support to patients, families and caregivers to achieve positive health outcomes. The peer health coach will help patients become independent, accountable and responsible for their health behaviors.
The peer health coach will expertly link participants to county resources such as faith community nurses, community centers and county safety net programs. The peer health coach will also assist patients with health center appointments and medication access. The health coach will also work closely with the wellness coach to prevent hospital admissions and readmissions by assisting patients with keeping medical appointments.
The peer health coach will facilitate effective and positive behavioral changes in the areas of physical activity, nutrition, stress management, and chronic condition self-management by using coaching, behavioral and positive psychology, counseling, motivational interviewing, behavioral change theory, appreciative inquiry, adult learning theory, and solution-oriented theory.
Peer health coach candidates will be lay people, preferably health center patients, who have experience in educating individuals on chronic diseases, caring for someone with one or more chronic diseases or self-managing one or more chronic diseases. They will have personality traits suitable for working with patients, such as being friendly, HIPAA compliant, caring, non-judgmental, and highly motivated. They will be able to communicate well orally and in writing in English and Spanish or French. Duties will include tracking encounters and other data as needed.

Current American Heart Association Healthcare Provider CPR is required
Experience in educating individuals on chronic diseases, caring for someone with one or more chronic diseases or self-managing one or more chronic diseases is preferred. High School diploma or GED required. Linguistic proficiency in a foreign language is required. Basic computer skills, including data entry and retrieval are preferred.
