Responsible for coordinating individualized patient care plans. Provides support to patients to ensure compliance with care. Coordinates services and functions as a guide and advocate in assisting patients to access both internal and community services. Coordinates follow-up with patients for missed clinical appointments, and works with them to overcome barriers to care. Ensures that patient’s appointments are scheduled and not missed. Assists in providing patient education appropriate to the needs of the patient.
High School diploma or equivalent required.
Must have excellent customer service, interpersonal and communication skills. Broad knowledge of health and community resources. Ability to work independently as well as with supervision and as part of a team. Demonstrated sensitivity toward the client population serviced.
High School Graduate
Healthcare education and/or training preferred. Outreach or customer service experience preferred. CPR certification preferred. New York State Driver’s License and vehicle preferred.
While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle objects; controls, reach with hands and arms; climb stairs; balance; stoop; kneel; talk or hear; taste or smell. The employee must occasionally lift up to 15 pounds. Specific vision abilities required by the job include close vision, distance and color vision, peripheral vision, depth perception and the ability to adjust focus.
