The Care Coordinator functions as a liaison between Primary Care and on-site Registered Licensed Social Worker.
Facilitates patient referrals for other community services.
Trains Medical Support Staff in the use of Mental Health assessment instruments.
The Care Coordinator
will develop an assessment tool kit which will consist of validated behavioral health screening instruments such as the Patient Health Questionnaire-9 and the Alcohol Use Disorders Identification Test-C, and will pilot these assessment tool kits on a subgroup of elderly patients at the Cobleskill Health Center, will educate and train the primary care provider team at the Center on utilization of the assessments.
Bachelors degree in Social Work or equivalent field of study
1 year of post-graduate experience with direct patient care in a clinical setting preferred.
Experience with the use of Mental Health Assessment tools preferred
Position requires high level of detail orientation.
Ability to work effectively with providers, nurses, patients and insurers.
Must be well organized and demonstrate the ability to follow through with assignments.
Ability to prioritize work, write clearly and perform work accurately is required.
Strong telephone skills are needed.
Basic computer skills needed.
Knowledge of health insurance helpful.
Knowledge of Community Support Systems
Position requires extensive sitting and regular walking.
