Growing Specialty Physicians Network that's part of a large Regional HealthCare System is looking for a Care Logistics Manager to collaborate with multi-disciplinary team. This care coordinator is responsible for monitoring and tracking of quality indicators. Facilitates effective and appropriate use of health care services by identifying high risk and high cost patients with complex medical and/or psychosocial needs, assessing short-tem treatment options and opportunities to improve the quality of care. Care Manager will also maintain effective working relationships with coworkers, physicians, case managers and other health care team members.
Major Responsibilities:
Conduct post follow-up on patients that have been discharged from the hospital
Completes variance reviews on quality measures
Perform chart review on claims data for Medicare eligible beneficiaries from various sources outreach to primary care practice.
Requirements:
Prefer a Bachelor Degree in healthcare field, public health, etc...
At least 2 years' experience in primary care setting
