Position Summary:
The Care Management Support Specialist provides administrative and scheduling support to the Director of Clinical Services, the Clinical Services Managers and the nurse care managers ensuring efficient, professional and confidential services. The Care Management Support Specialist assists the department in maintaining regulatory compliance with all activities. Support services include data entry, telephone communication with facilities and members regarding benefits and referrals, communication with the care management team regarding member and facility information, managing forms, monitoring admissions, discharges and transfers between care settings under supervision and documenting according to established protocol.

Key Functions/Responsibilities:
Answer incoming calls from facilities to review members’ benefits and eligibility
Tracking of service requests, completion of requests, and researching incomplete requests
Support administrative needs for Clinical Services as needed
Track sending and return of Care Plans sent for signatures including standard and PCA care plans
Communicate with facilities regarding member status and input information into electronic record
Communication with nurse care managers regarding members’ discharge status
Assist in preparation of documentation and meeting schedules for clinical rounds
Assist in generating departmental reports
Maintain and manage scheduling and tracking initial and reassessments for new applicants and members on a daily basis
Prepare assessment lists by nurse care manager on a monthly basis and provide to nurse care managers and per diem staff
Prepare necessary documents on members scheduled for assessment and provide to nurse care managers
Provide demographic information and driving directions to nurse care manager
Track status of assessments and obtain nursing documentation
Record and monitor status of state assessments and obtain outcome
Coordinate and submit results to Enrollment
Provide feedback to supervisor regarding assessment issues
Contributing time to special projects as required

Strong working knowledge of general office procedures and equipment, including copiers, fax machines, et al. Proficiency in Microsoft Office products
Self starter with demonstrated ability to function in a busy office environment
Minimum of two years relevant experience and a high school diploma, knowledge of medical terminology. Associates or Bachelors degree a plus, but not required.
