This position will cover cases in Audrain, Boone, Callaway, Cole, Howard, Montgomery, Randolph and Warren counties.
The Care Manager, as a professional, must be capable of functioning independently utilizing critical thinking skills by practicing good judgment and discretion in decision making regarding children’s safety and well-being. This position provides ongoing case management for a designated group of client families. Tasks include responsibility for Family Support Team facilitation, development of individualized care (treatment) plans, coordination of individualized services across all settings, coordination of providers working in the home, school, and community environment, documentation of support related activities and services. Specific duties include, but are not limited to the following:
1.Implement the Wraparound process with each assigned client and/or family, obtain guardian signature on Release of Information, facilitate Family Support Team meetings, write individualized care plan, arrange/schedule child and family support providers, and monitor on-going coordination of the care plan implementation.
2. Initiate and complete family/child assessments that address safety, risks, family strengths and needs, domestic violence issues and any other socioeconomic needs relating to sound treatment planning. Make decisions regarding permanency goals, necessary services, and make recommendations to the court.
3.Maintain face-to-face contact with each referred child and their caregiver once every week (may require extensive travel based on the individual needs of assigned child and/or family).
4.Maintain contact with family members as appropriate. Facilitate and/or supervise approved visitation between referred client and identified family members, especially parents and siblings, as requested by the Family Support Team and/or ordered by the Court.
5.Facilitate Family Support Team meetings while making decisions on the placement of the child.
6.Continually monitor and evaluate service planning with each child, family and other Family Support Team (FST) members as well as monitoring their activities and getting updates on each family from all service providers involved.
7.Participate on related Care Management teams and Case Reviews of the client case load.
8.Ensure quality of treatment activities by continuously evaluating the impact of services provided for the child and family.
9.Be available to meet the needs of the family (often outside of normal working hours).
10.Direct, plan, arrange, and/or provide crisis intervention as needed.
11.Manage the disenrollment process and assure a smooth transition to follow-up services and supports.
12.Facilitate permanency and family connections through use of family search and engagement techniques.
13.Provide opportunities for natural supports to be engaged in the child/youth’s life and participate in the care planning process.
14.Testify on behalf of abused and neglected children in court.
15.Assess risk of child abuse/neglect and ongoing safety of children.
16.Assess child and family functioning, protective factors, family strengths, family needs, stressors and coping mechanisms.
17.Engage families who may not want the service.
18. Evaluate the impact of services provided for the child and family and help them plan for and adjust to transitions.
19. Be knowledgeable of and follow Federal and State laws, as well as MACF and CD policies and procedures.
1.Direct coordination of child and family support providers.
2.Assure provider skills match those required by individual care plans.
3.Submit outcome data, satisfaction surveys, and treatment and service documentation to the respective Senior Care Manager.
4.Attend regular Care Management Team supervision meeting.
5.Maintain and update FamCare information and other required documentation.
6.Attend staff meetings, Family Support Team meetings, training, conferences, and court hearings.
7.Maintain client and staff files according to Alliance standards and as required by policy and procedures.
8.Complete continuing care and disenrollment packets, distribute to required parties, to the Senior Care Manager, and meet face to face with the relevant Children’s Division’s county service worker and supervisor, if requested, to review client status.
9.Plan and facilitate or co-facilitate Family Support Team meetings at least every 30 days or more frequently as needed.
10.Interact with community professionals as required including CD, CPS, MRDD, DYS staff, School personnel, and the DJO and other court personnel.
11.Coordinate the implementation of client’s individualized care plan.
12.Required to have a cell phone and use it to receive/make work- related calls in order to fulfill the duties of this position.
Provided semi-annually during the first year and annually thereafter by the Senior Care Manager and Care Manager Supervisor.
1.Master’s Degree in psychology, social work, criminal justice, guidance and counseling, or drug and alcohol with one year of experience in providing services to children & families; or
2.Bachelor’s degree in human behavioral science which includes 30 semester or 45 quarter hours either in development of human behavior, child development, family intervention techniques, diagnostic measure, or therapeutic techniques, such as social work, psychology, guidance counseling, drug and alcohol and child development and three years experience in the delivery of protective social services, family preservation services or targeted case management services in a public or private agency.
3.Ability to complete all required paperwork on time in a neat, grammatically correct fashion.
4.Computer skills in word processing and data entry
5.Access to a personal computer at home. Also required to have a company cell phone.
6.Knowledge of Missouri childcare agencies.
7.Current Missouri driver’s license, good driving record, vehicle insurance, and adequate transportation to escort clients to appointments and team meetings.
8.Freedom from communicable diseases.
9.Clear background check.
