The Case Manager will support and enhance the work of licensed clinical staff providing mental health and health care services to homeless and formerly homeless adults with disabilities living in permanent affordable housing sites.
Conduct outreach to identify and engage residents.
Conduct ongoing assessment of client needs, strengths, and resources in order to create treatment plan and provide appropriate services.
Maintain a caseload of 20-25 clients and provide intensive support services.
Develop and maintain working relationships with outside social service agencies, ensuring that clients and staff have maximum access to community resources.
Ability to assess clients' bio-psychosocial functioning.
Knowledge of psychopathology and treatment modalities for dual diagnosed clients.
Knowledge of and ability to use Harm Reduction and Motivational Interviewing techniques.
Ability to work effectively and independently in community based settings.
Demonstrated history of strong outreach and engagement skills.
EDUCATION AND EXPERIENCE
High School Diploma or equivalent required.
BA in social work, psychology or related field or three years relevant experience preferred.
