DESCRIPTION

Project SAIL (Support and Advocacy for Independent Living), a Ramsey county homeless services program is seeking a full-time Case Manager 895001. We assist chronically homeless adults with a mental illness to achieve their goals related to safety, stability and recovery. For chronically homeless individuals living on the street, the transition from the street to permanent housing is made in stages (as necessary) and staff work to create a service environment where individuals feel at ease, out of danger, with flexible program services to meet their needs. Through mobile support we work to develop relationships with the people we serve. We assist them with housing needs, physical and mental healthcare, and independent living skills.

The typical hours for the role are every Monday - Friday from 8:00am - 4:30pm.

DUTIES

This position will provide targeted case management services to chronically homeless adults suffering from mental illness and chemical dependency issues through housing and targeted case management. Our practices are derived from "Harm Reduction" and "Housing First" approaches.

The planned case load is 14 clients. Responsibilities include:

Documentation and billing for clients
In-home and community visits
Collaboration with collateral workers
Education and support of basic living skills
Support for maintenance of greater independence

QUALIFICATIONS

Required Qualifications:
Bachelor's Degree in Social Services field
2,000 hours supervised experience providing services to adults diagnosed with mental illness disorders
Reliable automobile
Current driver's license and excellent driving record
Valid auto insurance

Preferred qualifications:
Master's degree
Experience with homelessness
Understanding of long term homeless population
Knowledge of harm reduction and housing first
Experience with targeted case management
