The Case Manager Assistant works with case managers, providers, patient benefits advocates and medical provider staff to identify potential clients for the case management programs. A case manager assistant role includes: outreach, enrolling clients in Cornerstone database system, reports completion, monitoring and reviewing of case managers' caseload assignments in Cornerstone database system, etc. The Case Manager Assistant attends all required in-house and grantors meetings, trainings and/or conferences. If necessary, the Case Manager Assistant could be relocated from site or hours in order to assist the target population.

Requirements: High School diploma or GED equivalent and two years experience of data entry required, or Associate's Degree preferred. Experience working with the target population (pregnant women and infants). Bilingual (Spanish and English) and bi-cultural competency required. Computer skills required. Candidate must possess an Illinois driver's license, and automobile with valid automobile insurance.
