One Hope United is a social service child welfare agency that serves more than 15,000 children and their families in 4 states. Although there are countless child welfare agencies across the United States, very few organizations are able to offer the diverse array of prevention, intervention, and community-based programs and services available through OHU.

We are currently accepting resumes for a Case Manager II position for our Waukegan location . We are offering a competitive salary and a complete benefits package, including health, dental and life insurance; significant paid time off.

Requirements:
Degree Type/Area: Master’s Degree preferred.
Experience: Two years of related experience preferred.
Time Spent Driving: 15%

Specific Responsibilities:
  • Provide Intact Family case management services in Lake County for clients referred by the Department of Children and Family Services for supportive services.
  • Complete a comprehensive client assessment and strength-based client service plan.
    Assist and link clients with appropriate services (i.e. housing, food, mental health, education, domestic violence, medical services, etc…)
  • Conduct weekly in-home visits and ongoing client risk assessments.
  • Create and maintain detailed reports and case record documentation.
