The hospital case manager is a RN who coordinates patient-focused care in a cost-effective manner without diminishing the quality of care a patient receives regardless of age. The case manager offers alternative means of care to patients determined to be in need either because they are about to be discharged or require long-term care that is sensitive to the hospital's budgetary requirements. This person will advocate on behalf of the patient while working directly with administrators and physicians to determine the best course of action while communicating available high-quality resources to the patient.
EDUCATION: RN required
EXPERIENCE: must have 3 years of experience as a RN (Med Surg, Critical Care, or ED experience required
SHIFT: PRN Work as needed
