Position: Case Manager

Assignment: International Children's Program

Reports to: Lead Case Manager

Status: Exempt

Pay Grade: 10

Critical Action Items:
1. Responsible for coordinating case management services including the management and supervision of children's services professionals
2. Take reasonable action necessary to carry out the responsibilities assigned as long as such action does not deviate from established organizational policies and consistent with sound professional judgment

Measurable Deliverables:
1. Coordinate with federal agency for placement of residents
2. Complete Case Management Tracking for each assigned resident
3. Contact prospective sponsor to discuss role of BCFS and reunification process
4. Ensure sponsor information is included in family reunification packet
5. Attend weekly case staffing meeting with contractor field coordinator and follow-up with recommendations
6. Meet with each assigned resident weekly and additional meeting as requested by resident
7. Review completed family reunification packets for accuracy
8. Follow up with sponsor to discuss any issue identified in review of family reunification packet
9. Complete Release Request Worksheet, Case History (if applicable) and Verification of Release Forms. Submit along with copy of Family Reunification Packet
10. Receive approved Interim Verification Release Form. Review with child and obtain his/her signature
11. Prepare Sponsors for successful completion of Changes of Address, Changes of Venue form. Obtain child's signature. Send documents to federal agencies
12. Complete and submit notice when contractor approves release for child
13. Assist children during departure, as needed
14. Prepare, compile, submit, and maintain accurate records, files, forms, statistics, and additional relevant information in accord with agency policy, licensing and/or funding requirements
15. Review and sign off on client records in accordance with licensing, and contract standards
16. Participate in planning, coordinating, implementing, and evaluating designated social service program(s)
17. Participate in workshops, seminars, education programs and other activities that promote professional growth and development
18. Serve as Liaison with Consulates of residents in care
19. Other duties as may be assigned
