Community Educations Centers (CEC), is a leading provider of residential reentry and in-prison community corrections programs. This person will work closely with and assist personnel by working with them to ensure the proper referral of residents to all appropriate services available and necessary to them.
Duties may include, but are not limited to conducting intake and assessment for residents, scheduling appointments, making recommendations, coordinating services, compiling reports, keeping necessary records, and assisting other staff members in the performance of varied work duties such as referrals and after care planning.
Work is performed in accordance with established regulations, policies and procedures, but employees are expected to exercise significant initiative and independent judgment in discharging duties. Work is reviewed by the Facility Director or the Program Supervisor through an evaluation of the completion of assignments through independent and group supervision.
Establish and maintain referral relationships with service providers and correctional staff.
Arrange medical referrals for residents when applicable.
Arrange DPA, Social Security, food stamps and medical assistance appointments to ensure timely and continued receipt of benefits.
Arrange psychiatric evaluations and medication when appropriate.
Identify appropriate social service resources for residents and refer residents as appropriate.
Refer residents to appropriate vocational and educational programming.
Facilitate appropriate groups and individual sessions with clients.
Conduct outreach efforts to maintain full case load and facilitate referrals to residential programs.
Maintain contact with discharged residents until they have been established with other case management/social services.
Maintain a valid drivers license.
Other related duties as assigned by Supervisor.
BA/BS plus two years of case management experience.
