Title: Case Manager (existing)
Employment Status: Part-time
Regular Hours Worked: 24 /week Non-exmept
Description of Duties:
Identify drug/alcohol abusers in the criminal justice system with the potential for effective treatment outcomes.
Assess case management and treatment needs of each individual.
Develop and maintain productive relationships with community linkage.
Develop individualized service plans with each individual who is accepted as a client.
Transport clients as necessary, to apointment related and program goals.
Complete all required documentation, type correspondence and compile required statistics.
Maintain productivity as stated in program goals and objectives.
Refer individuals to appropriate treatment programs as well as other identified needed resources in the community.
Assess the treatment needs and the characteristics of each individual.
Collect supervised urine specimens from clients, utilize urinalysis, document results of urinalysis and arrange for bio-hazard material disposal.
Monitor progress of clients and serve as a liaison between referral source and treatment/community linkages.
Prepare monthly reports on client's progress for submission to referral source. In cases where the client is in treatment, report will be prepared with consultation from the treatment team members.
Conduct screens on potential clients who are incarcerated in the Allegheny County Jail.
Attend staff development and training as directed.
Minimum Qualifications: Bachelor's degree in Social Science/Human Services or Criminal Justice. Must be able to work at least one (1) evening per week. Must possess ability to administer the PA Client Placement Criteria (PCPC) assessment. Must be able to obtain clearance to conduct TASC screenings in the Allegheny County Jail. Must possess a valid PA driver's license and ability to operate a van.
