The Case Manager is responsible for managing a continuum of care from admission through discharge for assigned patients. The role reflects appropriate knowledge of RN scope of practice, current state requirements, CMS Conditions of Participation, EMTALA, The Patient Bill of Rights, AB1203 and other Federal or State regulatory agency requirements specific to Utilization Review and Discharge Planning. The Case Manager partners with the medical staff, utilizes scientific evidence for best practices, relevant data and compliance with the mission/philosophy, standards, goals and core values of Dignity Health (dignity, collaboration, justice, stewardship and excellence). Primary elements of the case manager role include: Care Coordination, Utilization Review and Discharge Planning.
. Three years nursing experience in an acute care hospital preferred.
. One year case management in an acute care hospital preferred
* A Masters’ degree in nursing with a concentration in Case
Management can serve as a substitute for the experience
. Bachelors or Masters degree in nursing required
LICENSE / CERTIFICATIONS:
. CA RN License
. CCM, ABQAURP or ACM certification(s) preferred
. AHA BLS Certification
. Fluency in English verbal and written skills,
. Working knowledge of Microsoft Office suite and work related software
. Bilingual skills preferred
