South Bay Day Services Case Manager
South Bay’s Partial Hospital Program seeks a Case Manager responsible for case management services, attendance and coordinating treatment for all clients admitted to the Day Treatment Programs.
Gain valuable experience as you work with our professional and innovative clinical team to provide clients with a full range of counseling services for their mental health and substance abuse recovery needs
Specific Duties will include:
Conduct in house as well as community outreach sessions with clients and collaterals to establish linkages with care providers in order to ensure continuity of care
Engage clients in the Program through monitoring of individual schedules, team and individual meetings to ensure appropriate treatment compliance
Develop new collaborative relationships within the community and represent yourself in a professional manner that adheres to all ethical and best practices.
Manage transportation request for all clients including referrals in addition transport clients that are awaiting approvals.
Setting up intake appointments at referring agency (hospital, state agency, etc.)
Maintaining competency in the field via continued education
As a Treatment Coordinator, you will need to be highly organized with solid time management skills as well as adaptable to a fast paced work environment. You will receive training after hiring
Specific qualifications Include
Bachelor’s degree in related field from an accredited educational institution and 2years experience in MMH
Master’s level degree in Social Work, Counseling, Psychology, or a Masters degree with a concentration in one of those areas : Rehab. or Vocational Counseling from accredited educational institution and 2 years experience
