The Case Manager facilitates patient care across the continuum with emphasis on specific patient populations to effectively manage patient needs within the organization's goals for length of stay and clinical resource utilization. He / she applies process improvement methodologies and data analysis to evaluate outcomes of care. The Case Manager performs medical record reviews for admission and continued stay criteria and ensures documentation accurately reflects acuity. He / she is responsible for discharge planning activities and collaborates with the physician and other healthcare disciplines so that care is coordinated and service excellence is championed. The Case Managers uses clinical expertise and problem solving skills to achieve optimal clinical and resource outcomes within effective time frames.
Case Managers must have 3 to 5 years of hospital based experience in the delivery of patient care as either a Registered Nurse (must be a licenced Registered Nurse in the state of Georgia)or Masters prepared Social Worker. This position requires excellent communication skills and ability to use word processing software in a Windows environment. Experience in Case Management, Utilization Review and /or discharge planning preferred.
