Provide case management services to DCFS wards with a mental illness in a TLP setting. Attend clinical staffings and other clinical meetings as necessary. Complete DCFS service plans and attend court dates. Assist youth to apply for benefits, obtain employment, further their education, tend to their medical needs, and strengthen their independent living skills. Complete documentation in accordance with Medicaid and DCFS requirements.
This person should have good written and verbal communication skills and strong basic computer skills, which include the ability to document services in a clear and effective manner. Experience in the mental health field is strongly preferred.
Must have a valid Driver's license and good driving record.
Must have 100,000/300,000/100,000 auto insurance.
