Program Summary: The Connecticut Rapid Re-Housing Program (CT RRP) is a statewide initiative designed to assist homeless households (individuals and families) as they quickly move out of homelessness and into permanent housing through the provision of time-limited housing supports and strategies with the ultimate goal of housing stability. The New Haven Sustainability Coordinator serves households who have exited homelessness into permanent housing with up to 12 months of home based case management. Responsibilities: • Acts as primary contact person for between 15 and 30 families/individuals residing permanent housing throughout Greater New Haven. • Provide the majority of services in the client's home or a location of the client's choosing.• Maintains timely and accurate client files, including Action Plans, correspondence and case notes.

  • Enter all client demographics, program enrollment information and other data elements into HMIS within 48 hours of incident. Responsible for accurate and up to date data entry for all program participants. • Locate and help secure appropriate housing and community resources for residents. • Act as "Critical Time Intervention" Worker for up 10 households residing in New Haven with moderate to high service needs.
  • Develop service plans for outreach with each client in caseload with signed agreement by client.• Provide assistance for households to increase income, accessing long-term housing subsidies, establishing and/or expanding family and community based supports, and assistance in increasing access to mainstream services.• Maintain positive communication with landlords serving program participants.• Contact households in client caseload at least weekly and make house visits to clients as needed.• Provide services for up to a maximum of one year or based on the individual household needs.• Develop needs specific budgeting and life skills training for each individual or family client.• Maintain strict confidentiality concerning any program participant's personal information including HIV/AIDS status. • Develop a strong community profile within the homeless services community as an advocate for clients.• Assist in program development and quality assurance activities as directed by Program Coordinator.• Attend required trainings programs and community meetings as assigned. • Attend regularly scheduled supervision, staff and or other meetings as requested. • Mandated to report any witnessed or suspected child abuse/neglect to the Department of Children and Families.
  • Assist with special projects and complete other duties assigned by Program Coordinator. Qualifications: Educationand ExperienceAssociates Degree AND 2 Years Human Services Experience or Bachelors Degree AND No ExperienceBilingual in Spanish, both written and oral required. Bicultural preferred. Ability to work with diverse populations.Excellent oral and written communication skills.Valid CT Drivers License, proof of insurance and access to reliable transportation.Experience in housing, housing law and other areas as they lend themselves to successfully enhancing this program.Highly motivated, creative with excellent interpersonal skills.
