Duties: JOB SUMMARY : Manages the aftercare planning of an assigned group of clients by collaborating with RN Case managers, therapists, and psychiatrists to complete details of the discharge plan and make modifications, and to facilitate the discharge of clients as soon as stabilized.
Implements and modifies aftercare plan for an assigned group of clients.Establishes and maintains contacts with community agencies and health care professionals. Works with clients and families in the discharge planning and implementation process.
Desired Skills: Interpersonal skills both verbal and listening necessary in order to communicate effectively with clients, families, unit staff, and community agencies.
Analytical skills necessary for evaluating the aftercare plan, recommending changes and modifications, and for carrying out the details of the plan.
Writing skills to be able to communicate effectively with aftercare providers.
Knowledge of and ability in the care and handling of patients in a specific age group (children, adolescents, adults, or older adults) as assigned is required, including consideration of the special needs and behaviors of that age group. If more than one age group is served, knowledge and ability specific to each age group is required.
Requirements: A Bachelor’s Degree in Psychology, Social Work, or related Human Services field. Must have valid driver’s license and qualify for Philhaven driving privileges. Must obtain PA drivers’ license in accordance with PA laws if license upon hire is out-of-state.
