The Visiting Nurse Association Certified Nurse Assistant (C.N.A) provides assistance with personal care to hospice patients; as directed by the Plan of Care. As an integral interdisciplinary team member, our C.N.A’s provide excellent care and service that is safe, timely, effective, efficient, and patient centered.
Our C.N.A’s provide assistance with personal care as part of the total plan of care including: activities of daily living such as bathing, dressing, feeding, and toileting. Assistance is also provided in meal preparation and diet reinforcement, prescribed exercises, safe ambulation, non-medicated enemas, preventive measures for skin breakdown, and assistance with positioning of the patient. Light housekeeping for maintenance of the patient’s environment and safety may be necessary.
All C.N.A duties are assigned and supervised by the Registered Nurse Case Manager. Any changes identified by the C.N.A should be immediately documented and communicated to the Case Manager. All written documentation is required to be completed per agency’s standards of timeliness, legibility, and accuracy. The C.N.A will also be responsible to attend Case Conference to give additional feedback on patients and obtain necessary knowledge to continue quality care.
High School Diploma or GED preferred
Successful Completion of a Certified Nurse Assistant program
Must be Certified by the State of Texas as a Certified Nurse Assistant; and in good standing.
VNA Qualifications include:
A minimum of 1 year experience in direct patient care in a healthcare facility or with a home health/hospice agency.
Must successfully pass a competency examination administered by VNA; approved by the Health Facility Licensure and Certification Division, Texas Department of Health.
Must demonstrate the ability to perform personal care skills with minimum supervision, communicate verbally and in writing, and lift patients safely with proper use of body mechanics.
Must possess the characteristics of courtesy, honesty, and integrity to meet the VNA expectations of quality care.
Must successfully pass a background check, satisfactory driving record search, and drug screen.
Must be able to show proof of the following items on the first day of employment:
- Valid Texas Driver’s License
- Automobile Liability Insurance
- A reliable vehicle (driving is required with this position)
- American Heart Association approved CPR Certification
- 2 approved I9 documents indicating eligibility to work in the United States
- Copy of degree/education or transcripts indicating successful completion
- A copy of current TB results or Chest X-ray if available.
