To be fully engaged in providing Uncompromised Safety, Superior Quality, Memorable Patient/Customer Experiences, and Financial Stewardship by: review of medical record documentation and assignment of clinical charges based on the defined acuity levels and review of documentation for completeness.

PRIMARY ACCOUNTABILITIES

Primary Accountabilities

  • Promotes understanding and awareness of Health First Clinical policies, procedures and practices
  • Verify that all patient charts are documented for proper services and are accounted for at 100%.
  • Assembles and analyzes clinical medical records ensuring proper charges are captured.
  • Reviews nurse documentation and provides feedback as it relates to charges to maximize revenue and patient charges.
  • May work closely with other departments and Materials Management to update and follow up on vendor contracts.
  • Routes reports and medical records to appropriate areas.
  • Accurately enters all clinical charges into computer system.
  • Reconciles patient charts and follow up with audits through Patient Business Services.
  • Accountable for effective routing of information
  • Charts are accurately analyzed, charges assigned and all codes entered into computer within the three days of service.
  • Accurately enter charges on the correct chart with a 98% accuracy rate.
Qualifications

High School Diploma required.

·

Minimum of 4 years healthcare experience

·

Three year experience entering charges for multiple specialties within a hospital.

·

Enters accurately an average of 135 charts on a daily basis.

·

Must be able to charge capture for at least three specialties with a 95% accuracy rate.

·

Familiarity with CPT and ICD-9

·

Thorough knowledge of medical terminology.

·

Advanced computer knowledge with proficiency in Word and Excel.

·

Excellent analytical and decision making skills.

·

Previous experience in healthcare or hospital setting strongly preferred.

·

Medical coding experience strongly preferred.

·

Excellent telephone etiquette.

·

Must have created one specialty training manual used to assist in standardization across the Health First System.
