The Chronic Care Coordinator will provide care coordination, case management, coaching, consultation and intervention to patients identified as high risk, or those having one or more chronic diseases. Responsible for working with the Care Center and/or the provider (Clinic, Physician Practice, hospital, or other entity) to identify the patient population for inclusion in services. The overall goal of the position is to help patients achieve and maintain a level of health and independence which prevents avoidable hospitalizations.

Qualifications

1. Current, active, and unrestricted registered nursing license or certification in a health or human services discipline that allows the professional to conduct an assessment independently as permitted within the scope of practice of the discipline required. Nurse Licensure Compact (NLC) documents the following: The mutual recognition model of nurse licensure allows a nurse to have one license (in his or her state of residency) and to practice in other states (both physical and electronic), subject to each state’s practice law and regulation. Nurses must legally reside in a NLC state to be eligible to have a multi-state license.

2. Baccalaureate or graduate degree in social work, nursing, or another health or human services field with appropriate licensure preferred.

3. Minimum of 3-5 years of acute care clinical experience; 2 years or more of care management experience preferred.

4. Experience working in a managed care environment preferred.

5. Case Management Certification (current or eligible) preferred, in one of the following:

a. Care Manager Certified (CMC)

b. Certified Case Manager (CCM)

c. Certified Advanced Social Worker in Case Management (C-ASWCM)

d. Certified Social Work Case Manager (C-SWCM)

6. Ability to communicate effectively with agency staff, upper management and other members of the health care team.
