Client Support Services Representative The Client Support Services Representative offers ongoing support and management of client cases through proper assessment, planning, implementation and overall evaluation of the individual clients needs.

Scope of Position: The overall goal of the position is to generate revenue and increase profit margins through increased client service contract enrollment/admissions as well as provision of different agency’s services.

  • To provide ongoing support and management of client cases through proper assessment, planning, implementation and overall evaluation of individual client’s needs.
  • To provide effective, efficient, adequate and appropriate delivery of private home care to patients. Has accountability for the care, coordination and discharge planning of all admitted clients.

  • Reports To: Executive Director

    Job Responsibilities:
  • Admits/evaluates/assess clients’ needs and explains the process of provision of home care at home as requested by the client.
  • Coordinates the integration of the different services needed and explains the client’s rights and responsibilities as well as the agency’s rights and responsibilities.
  • Coordinates the activities concerned in managing client’s needs and discuss the appropriate solutions to client’s issues and concerns.
  • Assures appropriate and adequate staffing to meet client’s needs. Ensures that staff assigned matches what client’s needs.
  • Adheres to agency’s goals, objectives, standards of performance, and policies and procedures.
  • Ensures compliance with quality patient care and regulatory compliance.
  • Coordinates the integration of services with other healthcare organizations, agencies, community resources and healthcare facilities.
  • Conducts supervisory staff visits as well as record reviews using specific indicators and criteria as approved by the agency, regulatory state and federal agencies as well as contracted payer sources.
  • Acts as patient advocate: investigates and reports adverse occurrences, and performs staff education as it relates to client’s needs.
  • Promotes effective and efficient utilization of agency’s resources.
  • Be fiscally responsible in the delivery of care.
  • Mobilizes resources and interviews, as needed, to achieve expected goal to assist in achieving desired client outcomes within the desired timeframe.
  • Conducts review for appropriate utilization of services from admission through discharge.
  • Evaluates patient satisfaction and quality of care provided.
  • Communicates with clients, family members, caregivers as well as physicians at regular intervals throughout home care admission and develops an effective working relationship.
  • Introduces self to patient and family and explains role and process for patient and family to contact client support services /marketing executive.
  • Assists and/or Completes assessment of patients and family needs at time of admission.
  • Enhances a collaborative relationship to maximize the patient’s and family’s ability to make informed decisions. Facilitates interdisciplinary patient care communication and/or conferences to review goals, optimize resource utilization, provides family education.
  • Collaborates with home health staff in the development and execution of the plan of care, and achievement of goals.
  • May be assigned with duties from time to time by Executive Director or Governing Body.
  • Participates in meetings as required by the agency or state/federal regulations.
