A. Education: Associate’s degree in nursing; bachelor’s degree in nursing is preferred.
B. Experience: 3-5 years of direct patient care or case management experience in a similar size, similar scope hospital preferred.
C. Certification/License/Registration: Current license as a Registered Nurse in the State of Georgia.
The Clinical Care Coordinator (C3) HUB acts as principal liaison between emergency services, surgical services, nursing, physicians, and utilization review to ensure appropriate bed placement is assigned, patient care needs are met and treatment plans are executed in the appropriate level of the care continuum. Facilitates patient throughput and coordinates with Unit C3s according to established milestones. Performs initial clinical review for Direct Admits to determine if patient meets criteria for admission. Facilitates the hospital transfer process for all facility transfers. Performs additional duties as needed to support the HUB, the care coordination model and clinical care coordination assistants (C3As).
