DESCRIPTION

Program Description: Fulton House is a transitional housing program targeting formerly homeless clients who are either mentally ill or dually diagnosed as mentally ill and chemically addicted. Funded and licensed by the NYS Office of Mental Health with additional funding from the US Department of Housing and Urban Development, Fulton House is located in Brooklyn and has a capacity of 48 beds. The mission of the program is to provide a safe and supportive environment where residents take part in rehabilitation interventions that will assist them in the attainment of their work, social and community living goals. A variety of services are available, including case management, substance abuse groups, medication monitoring, daily skills training and recreational activities.

DUTIES

POSITION: Clinical Coordinator, Licensed

PROGRAM/UNIT: Fulton House

HOURS: Full-time, 37.5 hours per week
Sunday - Thursday, 10:00am - 6:30pm

DUTIES/RESPONSIBILITIES: Supervise clinical staff in the provision of services; train staff on day-to-day program operations and protocols; work in conjunction with the nursing to ensure medication compliance; conduct regular chart reviews in compliance with NYSOMH regulations and quality standards of care; facilitate groups as needed; complete various monthly reports; attend case conferences; manage crises; network with New York City's supportive housing facilities for housing referrals. Provide emergency first aid/CPR assistance when needed. Related duties as assigned.

QUALIFICATIONS: LCSW or LMSW required. Experience with MICA population preferred. Supervisory experience, including Seminar in Field Instruction (SIFI) certified is preferred. Knowledge of Microsoft Suites and knowledge of electronic charting would be helpful. CPR training certification or willingness to take training class in CPR.
