Assumes responsibility for the overall nutrition management of patients. Uses The Nutrition Care Process to assess patients, identify patient-focused nutrition prescriptions and interventions and monitor and evaluate as appropriate. Re-assesses patients based on level of nutrition care complexity and Dept. policy. Works with healthcare team to determine required diet and patient education needs. Provides evidence-based nutrition education to patients, families, staff and the community. Interfaces with Diet Office and traypassing personnel. Performs daily patient rounds; appropriately screens patients based on established guidelines and Dept. policy. Maintains complete and appropriate records. Performs other duties as required.
: Bachelor of Science degree in Dietetics or Foods & Nutrition required.
At least two years of relevant work experience required. Position requires monthly participation in weekend work rotation and on call holiday coverage. Requires ability to cover vacation time as needed, typically during the summer and holiday times.
Registration with The Commission on Dietetic Registration (RD) required;
Certification as a Dietitian-Nutritionist (CDN) with New York State Department of Education required;
Membership in The Academy of Nutrition & Dietetics strongly preferred
