Reports to: Executive Director/Director of Patient Care Services
JOB DESCRIPTION SUMMARY
Responsible for establishing and maintaining positive relationships with customers and referral sources within the Community, responding to customer and Community needs regarding Community education as it relates to the hospice philosophy of care and delivery of services. Developing and increasing the access to Bristol Hospice Services in the communities that the location serves.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Ensure the educational needs of the community related to hospice care and services are assessed and met on an ongoing basis.
Continuously conducts community education needs assessments and develops an associated plan designed to meet community services as determined.
Establishes and monitors community, customer, and payer and patient perceptions of Bristol Hospice as a high quality provider of hospice services in the community.
Maintains comprehensive working knowledge in the field of community educational resources and shares information with appropriate organizational personnel.
Receives hospice referrals. Conferences with clinical supervisor and/or Director of Patient Care Services to collaborate and schedule intake activity.
Follows intake referral processes.
Participates in bereavement activities as assigned.
Participates in social work activities as assigned.
Serves as a liaison between facility, physicians, hospitals, skilled nursing facilities, and community and hospice program with coordination of referrals/admission process and services.
Promptly responds to customer and referral source requests and concerns.
Assists Executive Director and Director of Patient Services regarding the planning, implementation and evaluation of in-service and continuing education programs.
Actively participates in the Quality Assurance Performance Improvement (QAPI) process as directed by the Executive Director or Director of Patient Services.
Completes an initial social work assessment of patient and family.
Completes education on hospice service and philosophy.
Completes consent forms as assigned and indicated.
Associated documentation is turned into office according to administrative processes.
Participates in IDT meeting and works with the IDT to determine the patient/family plan of care as needed.
Provides patient/family education related to hospice philosophy and care.
Complies with accepted professional standards of practice and principles.
Provides other duties as required.
Graduate of an accredited school of social work. Social Worker shall have a minimum of:
One year experience as a professional social worker within the last three years : or
Have a Licensed Social Work Degree in social work from a accredited program.
Current LCSW licensure in State, CPR certification and valid driver’s license.
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist.
Prolonged or considerable walking or standing. Able to lift position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
Physical and Sensory Requirements (with or Without the Aid of Mechanical Devices)
Walking/mobility, reaching, bending, lifting, transferring and repositioning residents, grasping, pushing, pulling and fine hand coordination; ability to distinguish smells, tastes, and temperatures; ability to hear and respond to pages, ability to read and write; ability to follow written and oral instructions; ability to communicate with residents and others; ability to understand and apply training and in-service education; ability to instruct personnel during training education and staff meetings; and ability to remain calm in emergency situations and when handling multiple tasks.
