Serves as liaison between LTACH, referring physicians, and hospitals’ case management/social services, as well as other potential referral sources. Markets in assigned territory to build business relationships and develop referral base. Keeps current on healthcare market. Builds physician loyalty as referral source. Manages intake process. Completes electronic pre-admission assessment of patient. Collaborates with LTACH leadership regarding admission criteria of pending cases. Educates accounts, patients and families. Obtains payer decisions on pending cases. Coordinates transfer of approved patients. Promotes CHRISTUS Continuing Care mission and values in community. |
R.N., BSN preferred. Valid RN licensure. Current BLS certification. Basic computer skills. Effective communication skills. Energetic, positive attitude, team spirit. Sales and marketing experience, a plus.
