Clinical Manager, Care Management Practice Transformation Department Reporting to the Medical Director, Practice Transformation, you will play an integral role in helping lead our organizations efforts to transform patient care from a service-based practice model to a comprehensive, team-based model of care that will help us achieve higher quality outcomes at a lower cost of care. Responsibilities will include: Providing direction for overall care coordination activities Developing and implementing group-wide Care Management Plans and programs Overseeing, managing and developing departmental budgets Establishing performance standards, departmental goals, priorities and workforce responsibilities Planning population/utilization management goals Identifying opportunities to improve performance and impact cost of care Building programs that empowers and rewards staff Examining Care Coordination processes and implementing technology to improve effectiveness and increase productivity Monitoring performance metrics for risk/performance based insurance contracts and achievement of goals Providing daily direction, supervision and mentoring to Care Management staff Handling other related duties as assigned To qualify, you must have a current NJ RN License, BSN, and 5-8 years of progressively responsible clinical and management experience. The ability to implement and monitor plans to assess improvement of care coordination, the utilization of resources, and effectively support all efforts of practice transformation will be expected, as will the ability to prioritize and manage multiple large scale projects.

Strong medical, legal and insurance knowledge, along with solid decision-making, critical thinking, analytical, time-management, and oral/written communication skills are also essential.
