Functions to coordinate and formalize a multi-disciplinary structure and process that ensures accountability for specific patient care outcomes through assessment, planning, coordination and referral of services within specific time frames. Ensures continuous monitoring and confirmation of service delivery and evaluation of healthcare services for the patient and family. Collaborates with the multi-disciplinary team, utilizing clinical outcome data to support innovative and cost-effective changes to practice and to the healthcare system.
BSN required. Master's of Science in Nursing preferred.
Licensed as an RN in the District of Columbia.
Five years of relevant clinical experience.
Discharge planning/utilization review experience preferred.
Oncology experience helpful, but not required.
