Graduate of an accredited School of Pharmacy recognized by the California State Board of Pharmacy Licensed. An advanced degree such as a Doctor of Pharmacy or Masters of Sciences is preferred. Preferably two (2) years experience in clinical pharmacy or a residency program.
Provides consultative services including: anticoagulation, drug information, formulary reviews, in services on pharmacology/disease state pharmacotherapy, pharmacokinetics to medical, nursing, and allied health professionals. Provides prescribed medications, other pharmaceuticals and compounded medications according to professional standards and practices. Supervises intern clinical pharmacists and pharmacy technicians. Performs and supervises medication order entry, unit dose cassette filling, preparation of all sterile medications including chemotherapy, narcotic dispensing and accounting, enteral nutrition preparation, dispenses and prepares all investigational medications, participates in all code blues, and operation of the pharmacy information system.
