Responsible for the daily activities of the pharmacy programs. Assists with the development and execution of numerous initiatives that improve the quality or cost effectiveness of drug therapy in coordination with other care team members and physicians as identified.
Provide direct pharmaceutical care services to ambulatory care patients in a clinical setting and promote appropriate, safe, and cost-effective drug therapy to improve patients’ quality of life. Perform patient medication therapy monitoring and direct point of care patient medication therapy and disease state management counseling. Assist patients and physicians with medication profiling and necessary prior authorization as related to patient and point of care need. Assist in the development and presentation of educational materials. Handle phone inquiries from both internal (Markets, Specialists, Medical Directors) and external (Pharmacists, Physicians) customers.
All applicants MUST meet the following minimum requirements: PharmD or Bachelor’s degree in Pharmacy with advanced degree. Registered Pharmacist. Minimum of one year of clinical or managed care experience. Knowledge of computer applications, including pharmacy claims systems. Knowledge of regulatory requirements with emphasis on Medicare. Bilingual: Spanish/English
