A Clinical Pharmacist at West Side Community Health Services serves patients from many cultural backgrounds. You will work closely with providers, pharmacy residents and students to maintain a patient schedule and document activities which may include:
Diabetes management one-on-one and in group visits
Medication Therapy Management
Provision of drug information to our providers and patients
Coordination of patient assistance programs
Graduation from an accredited College of Pharmacy.
Current State of Minnesota pharmacy license.
Eligible to enroll as provider of Medication Therapy Management services through the Minnesota Department of Human Services.
Pharm D preferred.
Patient care residency/fellowship training preferred.
Bilingual (English/Spanish or English/Hmong) skills preferred.
