USC's School of Pharmacy is accepting applications for a full-time Clinical Staff Pharmacist with ambulatory care skills to rapidly develop and integrate comprehensive clinical pharmacy services into three AltaMed Clinics in Orange County. These services include comprehensive disease state management for the highest-risk patients (those in care transitions, frequent users of acute care resources, patients not meeting treatment goals for common chronic illnesses, etc.), medication therapy management for all patients (at minimum, medication reconciliation), drug information and medical staff education, and filling care gaps between the clinic and affiliated hospitals and pharmacies. The candidate will also have supervisory responsibilities for clinical pharmacy technicians, and will have teaching responsibilities for USC students and residents rotating through the clinics.

The candidate must have a Pharm.D. degree from an accredited (ACPE) school of pharmacy and be a registered pharmacist in the state of CA.

Completion of a postgraduate residency or equivalent experience is preferred. Prior experience in managing chronic diseases under protocol and/or experience setting up new collaborative practice models in medical clinics or offices is preferred. Bilingual English/Spanish is a plus.

Degree from accredited (ACPE) school of pharmacy. Registered pharmacist in state of California. Hospital pharmacy experience. Familiarity with unit dose, IV admixtures and clinical pharmacy practice.
