Provide initial intake assessment for individuals newly transferred to the team. Provide clinical assessment of all individuals assigned to the Safety Net Team. Assist in the development of Individual Action Plans (IAP) for designated individuals. Carry a caseload as assigned by the Safety Net Team Leader. Engage in relationship building with assigned individuals through direct phone calls and scheduled on-site and off-site meetings. Provide direct support and case management for assigned individuals including assistance with housing, rehabilitative goals, appointments, and other community living supports. Provide clinical training for staff individually or in groups as needed. Provide crisis assessment, planning, and treatment for individuals as needed at the request of the Safety Net Team Leader. Complete documentation pertaining to the client record, outcome measures, incident reports, and any other documentation assigned. Other responsibilities as assigned by the Safety Net Team Leader.

Qualifications:
LICSW, LCSW, LMHC, or other behavioral health category which qualifies as Licensed Practitioner of the Healing Arts. Three years direct care experience providing services to adults with mental illness. Valid driver's license. Proficiency in word processing and other computer functions. Demonstrated expertise in developing and writing assessments and treatment plans. Demonstrated professional communication and organizational skills. Computer literate in Windows, Word, and Excel. Ability to work effectively and collaboratively with a culturally diverse population of clients, other care providers, family and community members. Ability to work collaboratively as a member of multidisciplinary and cross-functional teams. Ability to function under pressure in a fast pace human service environment. Ability to be flexible, open and responsive to ongoing administrative changes, including being able to work independently with a minimum of supervision. Ability to articulate and communicate Bay Cove’s mission and strategic direction in a consistent and enthusiastic manner to departmental staff. Solid understanding of basic clinical and program model concepts.
